Summary
Coordinates/optimizes health care team interventions across the continuum and assists in developing and meeting outcome management and system improvement goals. Provides the Nursing, Utilization Management, and Social Work component to the case management triad and the management of patient/family care. Facilitates optimal reimbursement through accurate certification and complete chart documentation. Conducts concurrent admission and continued stay reviews based on utilization review criteria. Assigns working DRG. Prompts for documentation in chart to clarify diagnoses and medical necessity for continued stay. Provides psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family ability to optimally benefit from care.
Job duties

Coordinates care of patients across continuum in collaboration with triad and health care team.
Provides coordination of care consistent with accrediting and regulatory standards
Assists in providing processes/systems specifically for Identifying patients for service, assessing patients, planning, managing and evaluating care, continuum of care planning.
Performs select interventions/procedures, and interprets diagnostic tests within scope of nursing practice.
Assists in designing patient/family education tools/systems and guides nurses in the use thereof.
Requirements
Bachelor's Degree in Nursing required
Two years of experience as a case manager or as a RN in the assigned clinical area, with demonstrated clinical competence. This clinical experience must have taken place within the last five years of the nurse's career
Licensed as a RN in Colorado
Basic Life Support (BLS) through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire. All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer. (American Red Cross for the Professional Rescuer is only good upon hire and all renewals will need to be through an American Heart Association sanctioned course).
Specific clinical areas may require additional competencies such as ACLS, PALS, NALS or others as identified.
Preferred
Master's Degree in Nursing preferred
Two or more years as a practicing clinical Registered Nurse

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
